Question: Proper Wording for Medical ID

My son (who had myocarditis at age 2) needs to take penicillin prior to dental visits to prevent bacterial endocarditis. He also has an abnormal EKG and a coronary fistula. How do I put all this info on a medical alert ID for him to wear? Thank you so much for your help.

I'm not sure how many spaces are available on the ID's but you can boil it down simply with "cardiac issues, SBE proph."
If that doesn't fit you could even put "Heart, SBE proph."
All physicians should know what SBE prophylaxis means and if there is a designation that there is a heart issue they can look into it further to find out what it is.
