Question: Large and excessive scalp flakes

Male, 25

I just now combed and shook from my hair enough skin flakes to completely fill a tablespoon. And they're large, some 1/10th of an inch wide (I measured).

I've had a history of scalp problems (diagnosed by a GP with a "dry scalp" around age 8, dandruff from the onset of puberty), but this is quite different, and has gotten progressively worse over the last year. Previous conditions were treatable with coal tar extract and cider vinegar. Current problems are completely resistent to said treatments, and they seem to be worse after each treatment.

No hair loss other than that consistent with other men in my family.

Other present skin conditions are the occaisonal pimple, what may or may not be Keratosis Pilarus on upper arms and back (not as regular or thick in distribution as reference photos of KP... but that's been there since I was 10 anyhow), and some limited vitiligo on knuckles and armpits.

Not medicated in the last ~6 months, previously on 10MG Lexapro for depression.

This is really embarrassing, I'm constantly self-conscious if I don't have a hat on.

Thank you.

User avatar Your description is consistent with dandruff or seborrheic dermatitis. Typically selenium or coal based shampoos are sufficient to treat this condition.

You may wish to try a shampoo with an antifungal such as ketoconazole in it.

If this does not help I would suggest you see a dermatologist.

Best wishes.
