This reply obviously is belated. The history provided suggests an "acute scrotum". This is a surgical emergency unless proved otherwise.
This could be due to a number of causes including
- torsion of the testis,
- torsion of the apppendix of the testis(Appendix testis), and
- a strangulated inguinal hernia.
Some of the differential diagnoses are,
- an acute orchitis,
- an acute epididymitis or epididymo-orchitis,
- Acute idiopathic scrotal edema and
- insect bites.
I urge that any child with this kind of history should be taken immediately to the A&E department of the nearest hospital for an emergency assessment.Hope your brother was treated and is well by now.
