For the last 20 days I have been feeling some discomfort around my testicals. The pain is not inside but outside. I cannot wear my jeans because it gets worse. it feals like someone is peircing me. I visited a dermatologist he suggested an antifungal oral treatment with an antifungal cream. However there are no visible signs of anyithing down there.
Is there any suggestions of what to do since the medicine dos not help.

If, as you claim, there are no visible skin lesions on your scrotal skin, it is unlikely that you have a dermatological condition. Treatment for a fungal dermatitis would therefore not give any relief, and this is what you have experienced. Was a skin scraping done to confirm a fungal infection?
A piercing pain may be caused by a testicular pathology, so it is important to see a genitourinary physician or a surgeon to evaluate your complaint.
