Question: Severe itching and small hives
|bohemian - Mon Jul 05, 2010 6:04 am|
I am male, 30.
It started itching on my feet first a month before and then got worse day by day covering all body. Although it never showed any rashes/signs initially (red skin only when i itch and feels as if every pore to bleed). After doctor's consultation, I started using medicines, lot of them but none worked for me. Mostly now its on thighs and lower body. Also; it got making small but hard hives on thighs and soft parts of body those itch very bad during night. I am just sharing these pictures for clear view. Can be seen at
I have used these medicines so far. fexofenadine, hydrocortison, citrizine, famotidine

Hello Bohemian,Your photos show classic scabies nodules.Untreated scabies sometimes lead to such nodules which tend to itch at night. As tou may be aware scabies is causedby a mite which burrows in to the skin. This can spread from person to person.
The entire family woud need treatment.Do get confirmed the diagnosis since this is only a forum and we can guide you to some extent only.
Treatment would consist of permethin 5% application below the neck allover the body once a week for two weeks. The contact time would be 8hrs for the medication. The linen should be dry cleaned.Individual nodules may be applied with hydrocortisone creme daily till itch is relieved.
Hope this information is useful. With my good wishes.

Dear Dr.M.Jagesh kamath!
Thanks for your kind reply. That's exactly same what diagnosed by Doctor here in Saudi Arabia. Bad thing is that Permethrin is not available here. He suggested Benzyl Benzoate as an alternative. I have applied this lotion for three nights and feeling some relief. Can you please recommend some alternative for Permethrin? Any oral dose? How long should I continue the treatment? Can I use hydrocortison cream on gentile area safely for long time?

Hello Bohemian,Benzyl benzoate is fair enough.Remember that even the dead mite would incite a delayed hypersensitivity reaction, and this would not mean treatment failure or call for reapplication.
Oral ivermectin is good but the application does well too.Hydrocortisone is gentle in comparison to other topical steroids, yet the genital area is soft and absorbtion and side effects do occur when used long term.
My good wishes.
